Request for assistance forms must be filled out completely and can submitted to the Pay It Forward Foundation using any of the methods below.  We thoroughly vet every request for assistance by calling references, so please provide complete information in order to expedite the process.

With email, fax and online form submissions you should expect a call from a member of the vetting committee within two business days. With traditional USPS mail, you should allow an additional seven business days for a total of nine business days, to receive a call from a member of the vetting committee. If for any reason you have not received a call in the time frames listed above, chances are we have not received your form. Please call us at 1-888-732-5499 to discuss any issues.
